Pulse Audio Shortcuts - fixed

Adds a PulseAudio shortcuts menu to the Volume Menu. GS >= 3.4 can manage the displayed shortcuts via the prefs tool. The 'PulseAudio' menu text can also be modified via the prefs tool. (Credit: lmedinas, Xes)
